What is the focus of the Somerset Total Place programme?

The overall aim of the programme is 'to remove barriers to health and well being'.

 

This is an important step in reducing the dependency of individuals, families and communities on public services, as well as presenting opportunities to save money and to avoid burdens on public spending.

 

There is much evidence to show the current fragmented design of public services can perpetuate rather than break the cycle of dependency and cost more as a result.  If in the future more people need to take greater

responsibility for sorting out things themselves, then the way in which they are able to access services need to change too. Solutions will need to be found to avoid sending customers from pillar to post in person, on the telephone, or the net.

 

The programme will focus on three themes:

 

  • Transforming Community Services related to Health and Social Care.

  • Transforming support for families with the highest levels of need.

  • Transforming customer contact.
